Proof of death certificates: In Ontario, the Funeral Home choosen to provide services will issue will provide proof of death certificates. The Hodges Funeral provides an unlimited amount. Applying for the Canadian Pension Survivor's Death Benefit

The survivor's pension can be applied for by a person who is legally married or in a common law relationship with a person who has died. May also apply to children of the deceased who are under the age of 18 or between the ages of 18 and 25 attending school, college or university full-time.

Canceling an
Ontario Health Card and driver’s license

To cancel a family member’s health card following their death, you can visit a ServiceOntario Centre:

          St. Marys:  194 Queen Street West St. Marys ON N4X 1A4   
                             519-284-8137

       Bring the following with you:

  • the health card

  • proof of death certificate (issued from the funeral home

  • Print the Change of information form, complete sections A & F


For drivers license: Bring a proof of death certificate and the deceased’s drivers license
