Health Card

Ontario Health Insurance Plan (OHIP)-Health Card

The main office of OHIP in your municipality must be notified of the death in writing, by submitting a Funeral Director’s Proof of Death Certificate. The Health Card is to be returned with the Proof of Death Certificate, and then the deceased’s number will be cancelled.

As with any other extended health coverage, check with the local office and their representatives to see if the deceased belonged and if there may be a refund of premiums due. Also, inquiries should be made as to coverage for surviving dependant family members if required.
